Did the decapitation strike against the Ayatollah miss or not? Supposedly he's about to speak

Ok. This is starting to make a bit of sense.

Someone in some intel agency found out a meeting was going all, convinced the President to go for the strike, probably with the thought of a quick hit, end of fighting and to let the population topple the govt.

Was the intel verified?  Probably some popup shit so they had to roll the dice.  So if the goal was a quick hit to destabilize the govt and they got the location wrong or didn't use enough ordnance then that explains why we're here.

Could it be so simple?
